I'm a little late to this discussion, but here's my 2 cents.

For those that have never been an ISP requesting an IP allocation from
ARIN, you don't know the forms you have to fill out.  You have to list
_all_ of your web hosting by IP and domain name (as well as dialup
ports, IPs used for network infrastructure, etc.).  This is after having
SWIPed (a registration service) IP blocks you have assigned to your
customers.

Sometimes, ARIN will request additional information.  Some of this
information may include the same type of information you provided
originally, except from your customers (to verify your SWIPs).  This
isn't up to the ISP; you either get the info from your customers or you
don't get any more IP addresses.

In all likelihood, this is a request from the original poster's provider
that is being required of them by ARIN.
