Well, after many headaches here is the breakdown of what happened in case
this can help someone else.
I installed RaQ4-All-Security-2.0.1-15959.pkg and rebooted as instructed.
When I tried to refresh the web admin screen a few minutes later the server
wasn't responding (I was locked out but didn't know why). I couldn't SSH
either. The short version is that after the server rebooted it tried to
start up httpd which, of course, wouldn't come up because of the problem
with the pkgmaster php package.
Since there was nothing running on port 80, portsentry didn't like the fact
that I was trying to connect to port 80 so it dropped me into the hosts.deny
file. I added port 80 to the ignore list so if httpd dies again it won't
start locking people out for simply connecting to the web server.
After that I had to manually run the uninstall script for the pkgmaster php
package. Then I reinstalled the package and now everything seems to work.
I am concerned that reinstalling the pkgmaster php package broke something
that the security patch was trying to fix. But I am not sure how to test
this.
