Did you replace the right drive?
You may have made a simple mistake and replaced the good drive.
There has been a regular manufacturing fault in Qube3 production where the
IDE cables are crossed and hence the wrong disk gets reported as having
failed. Depending on how bad the problem was it is possible for the system
to boot from the "dead" drive and proceed to mirror itself onto the new
drive and then fail again.
Just a thought.
