I find that this is usually b/c of user error.  They forget that passwords are
case-sensitive, enter their email address alias instead of their username, or enter
username@xxxxxxxxxxxxxxx instead of username.  When you explain it to them over the
phone the problem is resolved and they usually don't realize they made a mistake or
don't want to admit to it.  I speculate that the fact that it works after you changed
the password is just a coincidence and has more to do with the instructions you
provided to the user and the user's better understanding of the steps involved.
