Sounds like the modification of the sendmail.cf has messed up sendmail.
Recover the file.

Maybe it never got to sendmail. Have you checked the form mail script (I
presume there is a form mail script). Add some logging to the script so
you can see when and where it loses the plot.

Sounds like the form script to me.
