any duplicates in any of the sendmail related files, including access,
aliases, etc. could cause this.

C9 is fine with me. Never updated with C7 though.  C9 supposedly fixed
issues with C6/C7 kernels.  OS restore is always last resort. If you
have spare server do Raqbackup and CMU to see if you can create users
after.
