Oops, my mistake. I meant it's not doing a POP on replies and only on
new emails.
I think your log shows just that:
First the attempt to send a reply

Then the POP login which should have come first:

I see a client of mine that always has two logins two seconds apart as
well.
Are you sure POP b4 SMTP is running?

That attempt came only 1 minute after the POP from the same IP so it
should have been allowed if POP b4 SMTP was running.
