Ok - so we set up the A record and the MX record, but
what entries should be in the Relay and Aliases boxes
on the email server and what should be the entry in
the 'Email Server Aliases' box on the site settings?
Sorry to go on, but we've tried every combination with
no success

You need to set up an A record for their mail server, eg
"mail.theirdomainname.com" which points to their IP address. Then you set up
an MX record which points to mail.theirdomainname.com.
That'll do it.
