I have put protected directories on maybe hundreds of servers, Cobalt,
Redhat, 
Mandrake, SuSE, and so forth.

I am 'almost' positive that it is not an issue with the file itself... As I 
have copied the file to another server and it works...
I copied the access.conf from a working server to this server, no joy!

Have you check that famous /usr/lib/authenticate file ???/
-rwsr-xr-x    1 root     root        18316 Aug  6 14:16 authenticate

I set mine for: chmod u-s authenticate and I could not authenticate?!
Just checking ;)
