 Are you sure it's POP before SMTP and not an open formmail.pl script
being abused?
Have you set up a user account and tried to send mail and then waited 20
minutes and tried to send again with using POP first? It's possible one
of your users gave someone a POP account and they're the spammer, or the
spammer figured out someone's POP password.

Without seeing some spam headers it's hard to say.
