Walk through /var/log/messages, /var/log/kernel
to see massages about PHYSICAL problem with disk.
If you have physical problem you should think
about replacing disk.
In case of some "sofware" faults, you can connect faulted
partition back by command:
# raidhotadd /dev/md4 /dev/hdc4
(according data you've sent)
