The only system I've ever seen where that sort of thing could happen "by
itself" was on an old Altos Xenix box where it simply ran out of inodes and
went off and started from one again thus splatting all the root files.
More likely this was some sort of disk partition file corruption fault where
it just went mad on the disk.
