Suggestions:

1) you said it is located in the 'root of the account'. That might be 
a problem if it is not in the 'web' directory

2) try deleting or renaming the default RAQ 'index.html' to something 
like 'index.sav' or 'index.def' if you want to save the file. then see 
what you get with www.name.com

3) when I first obtained my RAQ4i, I did the above: went into the \web 
directory, renamed 'index.html' to 'index.def', uploaded my 
'index.php' file, and www.name.com worked fine.
