I had some weird stuff like that too.  I got everything installed very
easily.  I went to create a new folder, and it created it, but it was
inaccessible.  So I checked the folders screen and made sure I was
subscribed to it, but still could not gain access to it.

Next I telnet'd into the box and ran pine, created another folder, logged
back into SquirrelMail and the new folder was there, and was accessible.  I
have no idea what I did to change anything other than create the folder in
pine.  I then deleted the old folder via the SquirrelMail interface.

After that, I was able to create folders in SquirrelMail just fine.
