It means that something is wrong with the access database.

You can check te exact error by doing.

/usr/bin/makemap hash /etc/mail/access.db < /etc/mail/access

there may be duplicated keys,  that you may need to remove from the access 
file.
If you do NOT have telnet access, then try checking directly on the e-mail 
relay domains or IPs box, and check for duplicated names or ips.
