I'd like to know the answer to this as well.

But there is another approach you might want to consider: software running
on the exchange server box which collects the email via POP as usual then
parses the header of each message to extract the destination address, and
pushes it onto Exchange as SMTP so Exchage can deliver it according to its
local setup. For an example see

Several of my customers have ETRN (also, apparently, also called SMTP
queueing) provided by large ISPs and have had a lot of trouble getting their
mail through. It seems unstable, probably due to the dialup server having to
recognise their static IP every time they dial up. The only time I have seen
Exchange doing this properly is when the Exchange server box has a 24/7
connection.
