I have done this (remotely too), but only after a fresh OS install. I
started 2 or 3 ssh sessions and then basically shutdown all processes
from /etc/init.d/. Then I created the directory /home2 and copied
everything in /home into /home2. (I used mc to do this, you can also
use cp -r ....). Finally I umounted /home and moved /home2 to /home.
Basically all I was doing was moving all secondary partitions to the
root partition. Obviously this is easier with a fresh OS.
Alternatively you could move the data to another disk, etc. Once I had
moved all partitions to the first partition, I used cfdisk to
repartition, paying special attention to not disturb the first
partition (or any other ones that had data).
Btw, I did all this because I increased the server memory to 256MB and
the original swap was only 128MB.
The above is basically an overview, if you would post the output of
these commands I (and others here) could be more specific.

df -k
fdisk -l /dev/hda
fdisk -l /dev/hdc
