I've seen this very same error many times when users put up
a FrontPage form that isn't properly configured. I'd personally
recomend forgetting about the system logs for now. I'd first open
the site in FronPage and check the parimeters for the form itself.
More often than not when I see this error it's the result of an
experienced user installing a form on a site using the new page
option but not configuring that form - primarily because they expect
it to work as it does when the form and the confirmation page
combination is installed via one of the site creation wizards.
