It means that named did a lookup for the IP of ns.mydomain.org.uk returned an
error.
Usually requested by services on your RaQ to determine if the IP address and the
domain name are authentic. It is usually difficult to tell if it is someone
spoofing
or if it is actually a lame nameserver, approximately 40% of all nameservers
are lame, is yours?
