I saw this happen a lot when I worked for a different ISP. It usually
happened when someone got cut off when they were doing a mail download. I
don't know how to really prevent it other than write a script that looks for
it and remove the lock files.
There are probably more reasons for it, but as dan said it might depend on
the patches and file versions you are running.

Might depend on what server and what patches you have or don't have.
