Yes to once per server... no to once per virtual host. The mysql runs as a
server on top of the server and each user on the server can use
it(certain restrictions apply)

you guve separate users 'accounts' by giving them privleges to use the
server. I'll email you a link to a nice explanation about how to add users
and their priveleges

I took the instrucitons directly from the cobalt site... I'll admit that I
felt unsure since I'm new to the server business... but I took them the
way that they said and it worked out just fine. there is the requirement
that you put one before the other, I think it's the php first...
