I think that most folks do set up via DNS.  

However, we have had very good luck setting up one
beige box that does nothing but MailScanner &
Spamassassin (we named the box "MailScanner"), then
set our Cobalts (a mixture of 550's and RaQ2s) and our
Plesk machines to run every message through the
machine we call "MailScanner" using iptables (or
ipfwadm on the RaQ2's.)

It works very well, and this way we do not have to set
up extra DNS for each domain we host, which is a good
thing, considering there are > 5000.
