This has worked well except for three vsites. When I test them in my
browser, I get a download popup (Open, Save, Cancel, yadda yadda). The file
name is index.html and the vsite URL is correct. But the index.html page
doesn't automatically open in my browser. If I select Open it looks fine.

This might be not the correct answer for you but I had the same problem a
year ago when I was surfing on the net.
What I had to do to stop the downloads of the index files was the following:
In the advanced settings of IE, I checked the User HTTP 1.1 through proxy
connection and the problem was solved.
Again, it's a wild guess but it solved my problem.
