Maybe it's because the recipient of a message presses the "reply to all"
button in his email client, and therefore two mails will be sent. One to the
address of the list, and a second one to the sender himself.

You can easily reproduce this error:

Just take an email from this list. Click on reply to all, and look what your
TO and / or CC field states. Ie my client adds to addresses to the TO field.
