Hey Jeff,
6 years of forms having no problem we finally had a similar issue but not
with verizon.net. The form info was being accepted but not passed on to the
end user. Just for kicks, we modified the cgi to send the info to a user on
the raq which had a foward to the correct end-user. The info went through
just fine. In the end we had to hardcode the e-mail header info- from (our
domain), to (end-user), subject (Internet Lead) etc... Any of these not
existing and the form info was lost. You probably have already been down
this road but I figured it was worth the mention...
