I do not have a how-to (yet). The main trick is to ensure the 
pop-before-smtp entry is in the .mc file you're compiling with m4 to 
make a new sendmail.cf.  A patch at some point wiped that out...  Or 
else manually add the entry back in once the new sendmail.cf is created 
(I think I posted the info the other day to the list).

I'll try to do up some more detailed steps and post them at some point soon.
