That's a very simplified way of looking at the problem, Dan.
The "To" address in mailings from almost all mailing lists is the name
of the list; the To address has nothing to do with email delivery.
Email delivery is based on the envelope recipient. The MTA (sendmail
for example) sends using envelope recipients, which are passed on to it
by your MUA (eudora for example). Your MUA gets the recipient
information for the envelope from the To, CC and BCC lines in the
outgoing message.
So when you send an email to the list, your local email client creates
an envelope recipient address from your To line, and the post gets sent
to the mailing list program.
Then the mailing list program forwards it (without adding any To, CC or
BCC lines) to every address on the list, by creating envelope senders,
which it passes on to the MTA (in our case, sendmail). Sendmail
bunches up envelope addresses if it can, so as to maximize bandwidth
usage, sending as many envelope addresses with one copy of the message.
And that's what nhs.uk is most likely blocking, based on the fact that
spammers do the same.
I consider it a poor choice on nhs.uk's part, but it's their choice and
their decision, and if Tim wants list posts to get to the users there,
he needs to solve it, perhaps by using another mailing list program,
perhaps by using a different MTA, perhaps by working with nhs.uk to get
them to whitelist his envelope sender address.
