I use a cleaned up redhat 9 version with adjustments for the raq4...
You can get it at

/usr/lib/apache/mod_auth_mysql.so
apache is not updated... you need to do that on your own using the sample
file (or file to include) located at /etc/httpd/conf/auth_mysql.conf
