Click the *gear* USER PREFERENCES icon.
Click the *envelope looking* EDIT PERSONAL EMAIL ADDRESSES icon.
Add an entry the way you want it to look.
Return to previous screen, USER PREFERENCES, and drop down the FROM list 
and pick the new entry you just created.
