i highly recommend http://www.spamshield.org/ - if you are familiar with
writing shell/perl scripts you can customise it.

mine is set so that if the same ip sends more than x mails in the last y
lines of maillog then block them for ten minutes (by blocking their route to
them it appears that the server has disappeared they can't see web or
anything) - you can use a white list too, for trusted servers like aol etc.
