I upgraded a customer server and received the same problem with no
descriptive error message. I then tried a manual upgrade through the Sun
Cobalt Knowledge base:

This gave a more descriptive error then a further
/etc/rc.d/init.d/sendmail stop (hit a few times to make sure) then a
start showed me a user had added duplicate entries into virtusertable
through non case sensitive users when the server can handle that (e.g.
someone had added "Test" and "test" as an alias for one account). I
cleared this through the Web GUI then ran the installation from pkg and
it went ok. Sendmail probably tries to rebuild the mail databases
through the upgrade but barfs on it if there is a duplicate entry.
BTW - I didn't get around to checking whether the server admin had
installed

since that should fix duplicate entries. May want to check that as well.
