I would make a small wager that the server was cracked.
Pull out the nonworking drive, mount on another system.
And take a look at it.
look for +i attr, check ps, ls, etcetera.
A client of mine had a server, where the web sites were working.
But you couldn't log in or anything. When I rebootd it, it
wouldn't come up. Damned if some cracker didn't try
to load mandrake linux kernel on it.
