We had exactly the same once.
This happened after we deleted a site and refreshed the browser instead of
using the navigation buttons on the site. The dataase then gets all confused
and deletes something it shouldn't.
I cannot remember the tool, but if you search the archives with your problem
you will find a very simply fix, something to do with meta-verify I think.
The script takes a few seconds to run and sorts it.
