The added entry into the file /etc/httpd/conf/mime.types was,
"text/x-server-parse-html (space) html".  Why, this actually 'broke' the
.htaccess file, I'm still not sure, but as soon as it was removed it worked.
Also, you stated that you compared the relevant files on both server's; was
this done for the .htaccess files as well?  Is the entry, "AuthPAM_Enable
off", in the .htaccess file that is not working?  If not, add the entry and
see if it parses the .htaccess file through.
