Hello Mike,
I had the same problem and I figured out why (Maybe it's
the same)
Problem:

I couldn't activate FP extensions with an error that the extensions
couldn't be installed. Same on the main site and users site. I've
tried to disable them (if any enabled) and re-enable them but without
success.
Solved

The problem was that I had previously uploaded via FTP a website
created with frontpage (with more than one web folder in it) and had
the same thing under the users' web.
1) I've prepared offline a new version containing only one Web.
2) Make sure all FP sites are disabled (Main+Users - for the website you
want to fix)
3) connect via SSH to the box and remove the whole contents under \web\
4) Enable frontpage extesions again (and this time should work)
5) Publish your website with the publish feature of FP
I've tested this with FP XP and it works fine.
