Will the secure.mydomain.com still have the same document root as
www.mydomain.com? If so, you don't have to create the subdomain user.

Correct. This only sets up a regular website running on port 80. If you want
it to be SSL you'll have to copy the a virtual host container from a SSL
enabled site and use that as your base.

As noted in the instructions, you'll have to create a couple of A records
for the subdomain.
