Here is what I've found in the error log

"cgi-wrapper" not found for: /home/sites/site9/web/cgi-bin/form_processor.pl

Are you sure that that's where sendmail is on your server as it's usually in
/usr/sbin/sendmail these days:

whereis sendmail

Have you checked your error log just after running the script:
/var/log/httpd/error
