You need to fix the following paths since they're incorrect for a RaQ:
> $form_info_path =
"/usr/local/etc/httpd/htdocs/demos/feedback_archive.txt";
> $email_datafile_path =
> "/usr/local/etc/httpd/htdocs/demos/feedback_emails.txt";
> $feedback_response_path =
> "/usr/local/etc/httpd/htdocs/demos/feedback_response.txt";
But the killer is probably this:
> #Path to sendmail
> $mailprog = "/lib/sendmail -t";
sendmail isn't there unless you have created a symbolic link. It's usually
in:
/usr/sbin/sendmail
