We've installed bulk_mailer with our own majordomo implementation; not
with the standard Cobalt install.

There are a couple of "standard" ways to install bulk_mailer along
with majordomo.

1. setting up an e-mail alias for the mailing list that pipes directly
   to bulk-mailer with output going to an alias for the list-out file.
 or
2. Setting up an e-mail alias that pipes to bulk-mailer with the output
   going to an alias for the mailing list-out file (an include)  and also
   setting up an e-mail alias for the mailing list that pipes to majordomo's
   resend with the output of the resend going to the bulk-mailer alias.

You could also set up a single alias with the output of resend being piped
to bulk-mailer. How did you do it? A solution involving resend is necessary
if the majordomo features that are implemented via the mailing list's config
file. I've even seen people set it up so that the output of bulk-mailer was
piped to resend.
