Hi Thom:
I have had similar problems. Did you make sure your client's directory
structure is cleared of all FrontPage extension files before re-adding the
extensions? I had to remove some of them manually, ie via SSH and then the
gui was able to add the extensions with no problem.

Have a user on a RaQ4 who emails me saying that Front Page no longer
works, and "Oh by the way, I deleted my own user account. so I can't
get into the server."
Quick as a bunny, I add him back in and attempt to get frontpage working
again(there is NOT www.hisdomain.com in /usr/local/frontpage) It looks
like it adds, but it doesn't.
I looked in the archives and don't seem to see a solution. Any
suggestions, short of wiping out the site and telling the user to start
from square one?
