Hi Mike.

This is how i have it set up on all my servers.

Create a sub domain called mysql and install phpadmin here.

So the path to your mysql for all users will be http://mysql.mydomain.com

Install a copy of MySQL on the server, set this up for http authentication
and then follow the guidlines at the following URL for setting up new
databases for each user.
