You should NOT have a problem with uppercase except for the initial
load. You can fix it either by adding INDEX.HTML and INDEX.HTM to the
list of allowable index pages, or you can just have a FILENAME.html and
FILENAME.HTML.
Unless your client is using subdirectories that should work. If he's
using subdirectories the easy way out would be to create symbolic links
from SUBDIRECTORY to subdirectory and allow symlinks for the same owner
within Apache for that virtual site.
If that doesn't work (it should), examples would be nice.
