Make 100% sure that you've selected "/web" and not only "web" - the slash
is very important. Using "web" will actually upload your files to
/home/sites/site21/users/userid/web, and not /home/sites/site21/web.
Another common cause is when uploading index.htm or index.php, and not renaming
the default index.html to something like cobalt_index.html. But I see you've
got that one covered.
