Correct, you do not need to restart sendmail if all you change is the
virtusertable (http://www.sendmail.org/virtual-hosting.html, #8 at
bottom of page).

On both RaQ3 and RaQ4, there is a line at the end of the
/etc/virtusertable (RaQ3) or /etc/mail/virtusertable (RaQ4) file:
# Put custom additions below (Do not change/remove this line).
Changes made below this line should not be overwritten/modified/etc
by the Cobalt scripts...

Or use built-in sendmail rule/address tests like 'sendmail -bt'
(then '3,0 user@xxxxxxxxxx' etc) or 'sendmail -bv user@xxxxxxxxxx'
