Are you creating the users via the GUI or via telnet/SSH?
If you're doing it via telnet - ie manually adding the entries to the
virtusertable - are you updating the virtusertable.db file using the makemap
command? Even if you're doing the changes via the GUI, it may be a good idea
to manually update the db file in this way, just to make sure.
If this doesn't work, send through the entries that you have in the
virtusertable, to give us more to work on :-)
