I got the same problem when i changed ISP, and the problem was that my old
ISP (that was the secondary DNS) didn't remove the entry from there record.
Hope it help.

http://mydomain.com points to me
but http://www.mydomain.com points to the old IP of the previous ISP.
They both pointed to me for about 1 week and then www. just started
pointing ot the old ISP again.
I've added the A records twice, and the site's IP is the same as the one
I'm pointing to. It all checks out. When I use nslookup with my
nameservers it looks up the IP fine. But when I use my upstream
provider (or any other) www. points to the old ISP.
What could the problem be? They've had 4 days to propagate since I've tried
re-adding the A records.
