I haven't looked at the referrer part of the original, but xFormMail allows
you to do just that -- hard code authorized domains/referrers into the
script, plus the author added some good autoresponse and flat file support.
There are a quite a few other variations on the same thing out there, five
pages worth if you search 'xFormMail' at Google (98,000 results if you
search FormMail)...
