Components fail.  People have posted about fans failing in the past, though
that really doesn't show how prevelant it is since people don't ever post
"My fan still works, thought you'd like to know."  You said "servers" which
leads me to believe it happened on more than one, but then you said "server"
so I'm not sure how many were affected.  If it was multiple servers you
might also question the cleanliness of the air in the facility and the
temperature that is maintained.  If it's just one server I'd get a
replacement fan and not worry much about it.
