I've got the custom error messages working on my R3 but could never get the
.htaccess file to access the password full path... I had to give it an
http://pathtofolder.com address.. which I absolutely hate, due to it
showing the folder/filename in the error message address box.. 

But then... all of that was before finding out about the AuthPam_Enabled
off part <g>

Has a really well detailed process about the changes needed to your
httpd.conf file
