If you send mail by hand via sentmail -t do you see the error? formmail if
barfing because it's calling sendmail and sendmail is spewing out an error
message where the headers shouls be, it helps to know what that error is..

one or both of Accept email, and Relay email would seem to be wrong...
My inclination would be to not accept email (since you aren't the MX
host) but to allow relay (because internal mail is effectively relayed
once to get out) but the raq4 may be different..
It may make a difference if the script is claiming a from address of the
internal hostname vs the MX hostname also...this will trip up relay
detection..
Here's a typical talking to yourself example (what formmail would be
doing)
